Here is the flag of Ziębice commune, in Z±bkowice county, Dolnoslaskie vojvodship, as presented at this webpage.
Flag adopted 5 December 2002, see U C H W A Ł A NR II/ 9/02 Rady Miejskiej w   Ziębicach z dnia 5 grudnia 2002 r. w sprawie Statutu Gminy Ziębice.

Flagdescription: three horizontal stripes of blue and white, proportioned 1:2:1; in center the communal arms.
CoA: azure a churchspire argent, charged with a window or, a gate or an eagle noir, roofed gules, between two castletowers argent 4x embattled; in chief dexter a crescent or pointing upwards, sinister a 6-rayed mullet or; in base a threemound vert.

Ziębice other flag

Znamierowski, in "Insygnia, Symbole i Herby Polskie, 2003", shows another (earlier?) one: blue, whith at top hoist a crescent pointing upwards, and a 6-pointed star, all yellow.
Crescent and star are from the CoA - but I have no idea what the status of this flag is.
