Here is the flag of the municipality of Třeština in Olomouc region (okres Šumperk) in the Czech Republic. It is published on this page, possibly adopted 9 June 2006 (although it is mentioned there as "navrh" - proposal).
Description: four vertical stripes of white and blue, proportioned 1:2:1:6; on the first blue stripe three yellow 8-pointed stars in pale; in the fly the white bottom of a fleur-de-lis, from which three yellow bullrushes arise.

Arms: per fess azure a demi fleur-de-lis argent emanating three bullrushes or; argent a plough-iron azure three 8-rayed mullets or.
